Client Portal

The Client Portal is for you to make appointments, reschedule, and complete the necessary paperwork.

After your initial phone call to Dr. Ramotar, and there is agreement to begin services, you may sign up for the portal to complete the initial paperwork, and fill out the client biography form.  Upon signup, the Informed Consent form and other policies to review will be automatically emailed to you shortly.  Please print and sign these emailed forms and bring them to your first appointment.  The initial appointment time/date will be agreed upon by you and Dr. Ramotar, but you can use this portal to rearrange and/or cancel your subsequent appointments without contacting Dr. Ramotar directly.  The client portal is completely optional, and provided to make scheduling a breeze!
